The completed form must be returned to Cheryl Tenney at the Lynnwood Police Department.  The following considerations should be adhered to:

1.      All addresses must be complete, including apartment number, city, state and ZIP code.

2.      All telephone numbers must include area code.  Both work and home numbers are needed.

3.      All criminal activity must be listed.

4.      Additional pages may be added as necessary to complete answers.

It is important to provide the full mailing address, with ZIP code, of all references, as well as both work and home phone numbers.  Please advise these people that a representative of the City of Lynnwood may be contacting them.
